Fees and Insurance

I am happy to provide a free initial phone conversation and consultation with clients who are interested in starting any of my services.

FEE STRUCTURE

For an individual diagnostic assessment | CPT code 90791 | 60 minutes at $175

For individual psychotherapy for 53 to 55 minutes at $150

For individual psychotherapy for 20 minute add on increment $50

For phone consultations between sessions at $75 for 15-30 minutes or

$150 for 30-60 minutes (please note that insurance does not reimburse for phone consultations or sessions)

For private or small group yoga sessions at $75-150 an hour

INSURANCE INFORMATION

I am an out-of-network provider all insurance companies. This means that my clients pay me a fee directly, I provide a “super bill” for services rendered (including all necessary information about codes and charges), and then clients can request reimbursement from their health insurance provider directly. It is your responsibility to contact your health insurance provider directly to understand your out of network benefits.

If I am an out-of-network provider for your insurance company and you will be seeking reimbursement from your insurance provider, please contact your insurance company before our first session and ask the following questions:

• Am I able to be reimbursed for seeing an out-of-network mental health provider?

• Do I have an out-of-network deductible I need to meet? If so, what is the deductible amount, and how close am I to meeting it?

• What is my co-payment and/or co-insurance rate for the following CPT codes: 90791 and 90837?

• Is prior authorization required?

• Do I need a referral before seeking mental health services?

• Is there a session limit?

• How much can I expect to be reimbursed for my sessions with an out-of-network mental health provider?

• How long can I expect to wait before receiving my reimbursement check from the insurance company?

Reduced fee options for psychotherapy services are available on an as-needed basis. As a general practice, 25% of my appointment slots are dedicated as reduced-fee. Please note that if you accept a reduced-fee slot, you will not be able to ask your insurance company to reimburse you for the session. If you are interested in one of these slots, please ask me about whether there is availability. Please also inform me if you are interested in paying a higher fee than noted above to offset the cost of those in our community who need a lower fee option.
